Job Scope:
- Facilitate Advance Care Planning discussions with patients and families
- Coordinate ACP documentation and ensure accessibility for healthcare teams
- Promote ACP awareness through education, training, and community engagement
- Monitor ACP outcomes, maintain reports, and support quality improvement initiatives
- Collaborate with multidisciplinary teams and community partners to enhance ACP services
- Support ACP events, projects, research, and programme development
Requirements:
- Bachelor's Degree in Nursing, Psychology, Social Work, Counselling, or related Allied Health discipline
