Care Coach (5.5 days office hours; East)

Responsibilities:
- Support the care team in coordinating and improving the patient’s care
- Work closely with other nursing and allied team to provide advocate care options and services to the patients and their family members
- Improve the patient’s care plan by understanding their needs
- Involve in community outreach programmes
Requirements:
- Degree in Social Work/Psychology
- Working experience in healthcare or social service sector is an advantage
- Ability to multi-task and work in a fast-paced and dynamic environment
