Responsibilities:
- Holistic Care: Support clients' emotional, social, and spiritual needs through casework, counselling, and group activities.
- Care Transitions: Manage intake assessments, respite admissions, and seamless discharge/placement planning.
- Crisis & Support: Conduct safety/risk evaluations and support families during difficult life transitions.
- End-of-Life Planning: Lead Advance Care Planning (ACP), legacy work, and provide empathetic bereavement support.
Requirements:
- SWAAB-Registered Social Worker with a recognized Degree/Grad Dip in Social Work.
- 3–5 years' experience in a hospital, community care, or eldercare setting.
