We are looking for a Case Manager who can bring people together, navigate complexities with confidence, and advocate for the well-being of persons living with dementia.

In this role, you will:
• Coordinate care plans to ensure residents receive holistic and person-centred support
• Partner with nurses, therapists, doctors and other professionals to deliver quality care
• Build strong relationships with residents' families and serve as a key point of contact
• Support admissions, care transitions and discharge planning
• Monitor residents' progress and facilitate care reviews when required
• Assist with audits, reporting and quality improvement initiatives related to social care services
• Maintain accurate documentation and ensure compliance with relevant standards
• Participate in multidisciplinary discussions and contribute to care planning
• Advocate for residents' emotional, social and psychosocial well-being
What we are looking for:
• Degree or Diploma in Social Work, Psychology or a related discipline
• Experience in dementia care, case management, healthcare or community care would be an advantage
• Strong communication and interpersonal skills
• Ability to work effectively in a multidisciplinary environment
• Proficiency in Microsoft Office applications, including Excel, Word and PowerPoint
• A genuine interest in supporting seniors and their families
More than managing cases, this role is about helping residents live with dignity, supporting families with confidence, and ensuring no one navigates their dementia journey alone.
