Case Manager

We are looking for a Case Manager who can bring people together, navigate complexities with confidence, and advocate for the well-being of persons living with dementia.

In this role, you will:
. Coordinate care plans to ensure residents receive holistic and person-centred support
. Partner with nurses, therapists, doctors and other professionals to deliver quality care
. Build strong relationships with residents' families and serve as a key point of contact
. Support admissions, care transitions and discharge planning
. Monitor residents' progress and facilitate care reviews when required
. Assist with audits, reporting and quality improvement initiatives related to social care services
. Maintain accurate documentation and ensure compliance with relevant standards
. Participate in multidisciplinary discussions and contribute to care planning
. Advocate for residents' emotional, social and psychosocial well-being

What we are looking for:
. Degree or Diploma in Social Work, Psychology or a related discipline
. Experience in dementia care, case management, healthcare or community care would be an advantage
. Strong communication and interpersonal skills
. Ability to work effectively in a multidisciplinary environment
. Proficiency in Microsoft Office applications, including Excel, Word and PowerPoint
. A genuine interest in supporting seniors and their families
