Key responsibilities:
- Provide client-centred physiotherapy in home and community settings to improve mobility, independence and quality of life.
- Conduct assessments of clients’ physical, functional and mobility needs .
- Develop and implement individualised treatment plans based on clinical findings and client goals.
- Provide evidence-based therapy to improve strength, balance, endurance and mobility .
- Educate clients and caregivers on exercises, fall prevention and safe mobility techniques .
- Maintain accurate and timely clinical documentation .
- Work closely with multidisciplinary teams to provide holistic care.
- Participate in case conferences, community outreach and health promotion programmes .
- Supervise therapy assistants and support clinical student placements .
- Participate in internal training and provide physiotherapy services across different settings, such as Home Rehab, Community Rehabilitation, Community Hospital and Nursing Homes , based on competency.
Qualifications and Skilled Required
- Diploma/Degree in Physiotherapy
- Fully Registered with the Allied Health Professions Council in Singapore
