As a Outbound Senior / Assisitant Manager, Healthy Precinct (Partnership & Community Development), you will be responsible for the following duties:

• Led the development of Healthy Precincts and Community Health Plans by collaborating with internal and external stakeholders, leveraging data analytics and insights to integrate health and social care services for improved community well-being.
• Established and strengthened strategic partnerships with grassroots organisations, social service agencies, healthcare providers, and community partners to foster collaborative networks and enhance community support.
• Facilitated cross-functional stakeholder meetings to align objectives, coordinate initiatives, monitor project milestones, and drive collective outcomes.
• Planned and implemented community health strategies, programmes, and initiatives in partnership with multidisciplinary teams and external stakeholders to improve population health outcomes.
• Monitored and evaluated programme performance using key metrics and outcome measures, ensuring continuous improvement and the successful delivery of healthier community outcomes.

Requirements:

• At least a bachelor's degree in any field.
• At least 5 years to 8 years of relevant working experience.
• Able to think critically and strategically, with strong conceptual and analytical abilities.
